  • Step 1: Get ready to register. Have your Attendance Record (AR) or Provider Payment Request (PPR) form with you because most of the information you need to register is on the AR or PPR
  • Step 2: Have your Tax Payer ID Number or Social Security Number ready. If you have both, we need the number you use to do business with Crystal Stairs (the number on the federal W-9 Form in your Provider Contract File)
  • Step 3: You will also need to have an e-mail address. If you do not have one, you can apply for one through one of the web sites listed below.
